APPOINTMENTS

All visits are scheduled by appointment.  Each appointment is scheduled for 45 minutes.  Out of courtesy to the next patient, I will end all sessions on time, even if you arrive late.  If you are unable to make a scheduled appointment, I request that you provide at least one week notice.  Please note that insurance does not reimburse the fees for missed appointments.

PAYMENT POLICY

Payment is due at the time of each session.  You can settle your bill by personal check, cash, or Zelle.  You will receive a statement at the end of each month with a list of all services performed and payments received.  This office does not contract with any insurance carriers.  Your invoice statement will include all of the information necessary for you to submit an out-of-network claim to your insurer for reimbursement. 

COMMUNICATION

If you need to reach me between appointments, please call my office at (312) 620-0409.  I will do my best to return your call within one business day.  If your call is urgent, I will return it within two hours whenever possible.  Because communication via email and text message is not secure and may compromise your privacy, I request that you only use email or text message for scheduling matters. Do not use email or text message in the event of an emergency or to communicate confidential information.

PRESCRIPTION REFILLS

Please allow two business days to obtain a prescription refill in the event you will run out of medication before our next scheduled appointment.  If you need a prescription for a stimulant medication, please allow one week.  To obtain a refill, please call my office at (312) 620-0409 and leave a message with your full name, birth date, medication name, and pharmacy name and address. 

EMERGENCIES

In the event of an emergency, please call (312) 620-0409, and I will return your call as soon as possible.  If the emergency is life-threatening and cannot wait, call 911 or proceed to your nearest emergency department.  If I am away or otherwise unavailable, a colleague will be covering all calls.  Do not use email or text message in the event of any emergency.
